I have a question. Does ATP give the Training Materials or I have to get it myself?

Neither, the Flight Training Bundle is for after you have secured a class date to start the whole program. The $195 is for the Admissions Flight.

Do we include the mid-air refuel after take-off or just the running start point to point time?

We strive for 100% accuracy here on Airline Pilot Life, so it is necessary to include the refueling time as well.
