Hey! (Sorry, didn’t know where to put this question.)

I received my medical and very stupidly cut it incorrectly. In particular, the part I cut out was solely the medical portion of the certificate; the limitations and the portion at the bottom are no longer attached. Should I order a replacement?

Also, I tried to get a replacement via the pdf form on the FAA website; however, there is no option for medical certificate. There’s only the airmen certificates.

Anyone know how to help? Thank you

That is where I went to. When clicking the pdf for the application, there is no option for the medical certificate.

There’s only one form. For type of certificate you need to write Medical and whatever class you have.
